VIRGINIA DECA 2006-2007 Officer Training Conference (OTC) Form

• Complete the online form below.
• Pay close attention to your school code.
• Type the title of your officer exactly as it you want it on their name badge.  For consistency's sake, please use the phrase "District Advisor" or "District President" if it applies to you or your student.
• Select which conference you plan to attend (RO=Roanoke, RI=Richmond).  If you have some members attending Roanoke, and others attending Richmond, you may use the same form and register all students simultaneously.  Just be sure to properly identify which conference they will attend.
• If you have students that are attending both conferences, please enter their information twice, changing only their conference location.
• Registration fees are $12.00 for each student and advisor.  You may register one primary district advisor and one primary district president at no charge.  Please identify these people in the the "Primary DP or DA?" category.
• Do not use commas (,) anywhere in your entry.  If you feel the need to use a comma, please consider what other form of punctuation may be used.
• Enter your participants in whatever order you choose.  The data will be resorted once we have it.
• Check spelling for correctness!  This is critical, as this is the spelling that will go on name badges.  IF YOU TYPE ALL LOWER CASE LETTERS, THAT IS WHAT WILL BE ON YOUR NAME BADGE!
• Click the "submit" button.  Return to this screen and enter more membership data, if necessary.  There will be a link back to this page after you submit your data.  Continue this process until everyone from your chapter has been registered.
