All forms and other written requirements should be delivered in hard copy format no later than the date above. (Off-campus students may e-mail most documents--excepting the performance appraisal--in lieu of delivering hardcopies.) Early submissions are welcome. The performance appraisal must be signed by your supervisor. Make arrangements several weeks before the due date, as "My supervisor was on vacation" is not a viable reason to not have it signed on time.
